At Poland Medical we value your privacy and we are following the rules set by the General Data Protection Regulation (GDPR).
This Policy provides basic information for the htpps: //polskaprzychodnia.co.uk website users about the purposes, processing and the security of personal data and about the cookies.
Processing of personal data is based on the European Parliament Regulation 2016/679 from 27 April 2016 on the protection of individuals with regard to the processing of personal data and on the free movement of such data, and the repeal of Directive 95/46 / EC (General Data Protection Regulation).
Obtaining information about the website users and their behaviour is carried out as follows:
- through data entered voluntarily by users via the electronic form on the subpage / contact
- by saving cookies on end devices (own and third-party cookies)
- by collecting web server logs by the hosting operator
We only collect information voluntarily provided by the users, although information about connection parameters (time, IP address, browser) does not require users' consent.

Personal data may be disclosed to the service providers for the Administrator, who are responsible for the operation of IT systems, equipment and online services as well as marketing agencies.
When using the contact form provided on the polandmedical.co.uk website, we use the Google reCAPTCHA mechanism. This is a security feature to allow only human-filled data to be sent. Captcha stands for Completely Automated Public Turing test to tell Computers and Humans Apart.
Administrator reserves the right to disclose selected information about users to state authorities or third parties who submit a request for such information, based on a relevant legal basis and in accordance with applicable law.

Consultations, stored data
Our patients' medical records may be processed electronically, on paper, or both. In any case, thanks to the implemented work standards and available technology, we can ensure that your data is confident and properly secured. Data includes information we collect about patients, provided to us directly by patients or obtained from other sources (e.g. GP, hospital or laboratory).
Data stored by our surgeries include the following:
- details of the patient, such as name, surname, date of birth, address, contact details and details of the next of keen
- scheduled appointments
- notes made about the patient's health
- treatment and follow-up details
- test results, e.g. laboratory tests, X-rays, etc.
- additional relevant information from other healthcare professionals, family or people caring for the patient
All medical information stored in our clinic is confidential and may only be disclosed to people involved in the treatment or care of the patient.
Personal data may be disclosed in the following cases:
- when legally required
- with the patient's consent - given directly by the patient or taken for their best interest (or for other specific purposes)
- for reasons of public interest
You have a legal right to apply for access to your medical records. You do not need to give a reason. You can either view them or have copies issued by requesting this in writing and providing proof of identification. A request for your medical records should be made directly with our surgery. Please ask receptionist for details.
Online Payment System
Due to the Coronavirus pandemic outbreak and related restrictions we offer doctor consultations via Skype. Payment for the Skype consultation (in full amount) must be made in advance, at least one day before the consultation so that the payment can be approved in time. Prices can be found on our price list.
You can use BACS payment. You can call the surgery registration (0208 9034874) for all the details regarding transaction (our account number and sort code).
Skype consultation - cancellation and transaction refund policy
We have to process personal data in order to respond to your refund or cancellation requests.
In such circumstances we will process your personal data in accordance with our contractual and legal obligations with you and our legitimate interest to provide you with good customer service.
All transactions are eligible for a refund if you would like to cancel a payment for any reason within 24 hours of your transaction and 24 hours before your scheduled consultation.
If it is necessary to cancel your scheduled consultation, we require that you call at least 24 hours in advance.
A late cancellation is considered when a patient fails to cancel their scheduled appointment with 24-hour advance notice.
No refunds will be made for late cancellations and the full amount of the consultation will be lost.
Refunds will only be issued for the cancelations made 24 hours or more in advance. The refund will be made in the same form payment was made. Please allow 7 working days for processing.
No Show Policy: A “no-show” is someone who misses an appointment without cancelling it in an adequate manner. A failure to be present at the time of a scheduled consultation will also mean that the full amount of the consultation will be lost, and you will not receive a refund.
You can change your Skype consultation date after you've made the payment. The payment will be transferred to your new Skype consultation date. However, we ask for a possible early notice about the date chage, and failure to inform us at all will result in loss of the payment.
Outpatient consultations
We don't require a deposit for booking a consultation in our surgery. However, we kindly ask you to inform us if you cannot attend your consultation for any reason. Failure to inform us about your absence has a negative impact on the work of our doctors and the entire surgery. It also affects other patients. We have the right to refuse future consultations for the patients who didn't attend their consultation without prior informing us.
Cookies
Cookies are small files that are placed on your computer by websites you visit. It’s a way for a website to remember you, your preferences, habits, and possible changes made by you online.
